Forms
The Wendt Center for Loss and Healing offers forms electronically for current and previous clients to fill out. Please make sure before filling out any of these forms that you have been directed to do so by a current staff member, so that we can make sure you are filling out the correct forms.
Release of Information Form
This form can be used to allow the Wendt Center to provide information about your services to another individual, organization, or entity.
Office and Virtual Teletherapy
If you are already enrolled in the Patient Portal, you should have received an email about any forms you may be asked to fill out. Please try logging in to the patient portal before signing the form via our website.
